Questionnaire to be completed by a current or former student, family member, fellow teacher or supervisor. Regardless of who submits the original form, it must also be completed by both an educational peer and current supervisor.

1. Instructional excellence

Share how you know the nominee and for how long and include examples that demonstrate the nominee’s excellence in classroom teaching through use of engaging age-appropriate instructional strategies, technology and differentiated instruction to meet diverse learner needs.

2. Student engagement and classroom environment

Share examples of how the nominee establishes a safe, inclusive classroom that engages the students, promotes critical thinking, curiosity and collaboration and fosters students’ social-emotional development.

3. Professionalism and collaboration

Share examples as to how the nominee has earned respect from colleagues, families, students, participates in professional development activities and goes above and beyond to support student success.
